On this page, you will find an explanation of the documents you need to complete your application. There are documents that everyone must submit and documents that you only submit if they apply to you. For each document there is stated whether the original, a certified copy, or a regular copy must be submitted.

Which documents does everyone need to submit?

A certified original diploma or a certified copy of the diploma

This can be a certified copy or a copy with an authenticity code of your EEA diploma. A certified copy is a paper copy of your diploma bearing an ink stamp and a signature or initials from your educational institution, or from a notary. Most educational institutions provide one or more certified copies with the original diploma. Requesting a (new) certified copy of your diploma may take some time.

A certified diploma supplement (also referred to as a Syllabus or Curriculum)

This is a clear and complete overview of the content of the professional training program you followed. The supplement must indicate your full name, date of birth, and a comprehensive overview of the courses or subjects completed as part of the programme.

Curriculum Vitae (CV)

The CV contains your personal details and all your education, courses, and work experience. For each section, please include at least:

  • The start and end date
  • The country and place where you worked or completed the education or course.
  • The CV must be complete. There should be no gaps of more than three months. Have you not worked for a while? If so, please indicate this in your CV.

The document can be uploaded in your application and does not need to be certified.

Copy of your Identity Document

A copy of your passport, identity card, or residence permit does not need to be certified. A driver's license is not valid. Ensure that your full name, date of birth, and expiry date are clearly visible on the copy. Make a secure copy and cover any other personal data. You can use the Central Government's KopieID app . The document may be uploaded.

Statement(s) of no restrictions of authority

A document proving that no measures or restrictions have been imposed abroad that prevent you from practicing your profession. In some countries, this is also referred to as a Certificate of Current Professional Status, Letter of Good Standing, Attest, or an (Unbedenklichkeits) Bescheinigung. The statement needs to be a certified original or a certified copy.

You request the statement in the country where you obtained your diploma and in every country where you have worked as a veterinarian for more than three months.

Are you not (or no longer) registered in a professional register?

Then you must also be able to demonstrate that no measures or restrictions have been imposed that prevent you from practicing your profession.

You do this by requesting a Certificate of Non- Registration in the country where your diploma was obtained or where you worked for more than three months. This document must be an original or a certified copy. The document must not be older than three months.

Where can you request the statement?

On the list of competent authorities, you will find the foreign organizations known to us from which you can request a declaration or obtain information about this. You can also view examples of documents that you must submit from abroad.

If a country does not issue declarations or if you experience problems obtaining a declaration, you must demonstrate this by, for example, submitting your correspondence regarding this with the competent authority.

Additional statements regarding your diploma

If one of the situations below applies to you, you must submit an additional document with your diploma (onderstaande onderdelen worden uitklaponderdelen).

Are your documents in a language other than Dutch, English or German?

Then you must add both the document in the original language and an original translation. The translation must be done by a sworn translator. If you send a copy of the translation, you must have it certified by the authority that issued the document or a notary.

Please note: the translator cannot authenticate the translated original document. This can only be done by the institution that issued the document or by a notary.

Preferably send certified copies of your original documents.

You may send the original, but due to the risk of loss, we advise against this.
